Settlement Information for Class Members

United States District Court Judge David H. Coar has granted a final extension of the deadline to file claims to March 31, 2008. This is the absolute, final deadline for your claim form to be received. After March 31, 2008, any remaining funds will be revert to Cook Conty. If you have not received a claim form, or if you want to check on the status of your claim, please telephone the Claims Administrator at 773-896-9016.

Based on the number of claims filed, each class member will receive $200. Payments should be mailed by the end of December, 2007. Class counsel expects that claims received after November 22, 2007 and before January 11, 2008 will be paid at the end of each January, 2008.

Your claim form must be received by March 31, 2008. Telephone the Claims Administrator at 773-896-9016 to check to see if your claim form has been received.

YOUR OPTIONS AS A CLASS MEMBER:

  • SUBMIT A CLAIM FORM: This is to only way to get a payment. Complete and return the claim form you received in the mail no later than March 31, 2008. If you have not received a claim form, please telephone the Claims Administrator at 773-896-9016
  • YOU MAY NO LONGER ASK TO BE EXCLUDED FROM THIS CASE: The deadline to opt out of the case has passed. You therefore no longer have any right to opt out, which means to exclude yourself from both the class action and the settlement.
